Functioning true hermaphroditism isn't even possible in humans, and even less possible in terms of the symmetry and visual appeal that is seen in most futanari artwork. There's no one to really offend, and the only group that could reasonably take issue with it is people who are intersex.

actually functioning hermaphroditism is possible in humans, however the 1-1,000,000 chance of it happening and doctors not making the baby a male or a female is pretty low, theyre supposed to ask the parents what gender they want the kid, its called "gender mosaicism" it means the baby is born with full functioning male and female parts and if not surgically removed they will grow up to have functioning parts and can bear children and impregnate a female
