There is absolutely no reason for the condoms on the left thigh to be tied in this way, unless that leg is going to be permanently lifted upwards. It can't be, because that would lead to ischemia of the leg.
That means someone deliberately tied the condoms upside-down, causing him/herself much more trouble than necessary. And despite all of this, the artist got the right leg straight.

They're tied off - rotating the leg causes the filled end of the condom to point downwards regardless of starting orientation.
