Yeah, Red TOTALLY has Satoshi's birthmarks (the Zs), which is the only distinguishable physical trait between Gen I Red and Satoshi.

I'm calling it, it's Satoshi. Considering it's the anime version of Kasumi that's fantasizing and the preggo Kasumi is wearing her game outfit, it's probably Satoshi with Red's Gen III outfit slapped on. Again, black hair and birthmarks on his face. The "I knew he liked me the best" line (Kasumi/Hikari/Haruka) doesn't even work with Red.
