Being brother and sister absolutely does not guarantee problems with the baby. It increases some risks, sure, especially if the family already has some real health concerns, but a brother and sister can have perfectly healthy babies. It's happened many times, and a lot of you have incest in your family tree whether you know it or not.

The real problems occur after generations of incest with the generational gap shortening the closer the family members are related. The son and daughter of a brother and sister wouldn't want to have babies without checking for potential problems. Recessive genetics are a much greater concern in that scenario.
