Theoretically all one would need to do is take an existing stringkini and do a bit of cutting. Would be a simple matter to use some of the back piece for a neck tie used for obvious adjustment to remove slack. Then use the remaining for the desired amount for your anal desires. 1" wooden crafting beads arent hard to come by or a little woodshop knowledge/tools you can make your own if you want to go larger. After that its just a matter of mounting the bead(s).

Seal the anal bit in a little latex or rubber if you want something easier to keep clean if you arent the sort to keep your booty hole cleaned out.

I would do it myself but I neither have tools, crafting experience (I was terrible at HomeEc, okayish at woodshop), nore even know a person who would wear an unmodified version for me to want to pursue the tools and experience. Though I suppose if I was really serious I'm sure Craigslist could help me find someone interested.
