I know it says commentary_request but the story behind this is a bit hard to translate. Let's see:

Kappa are said to gain power by taking their victim's shirikodama (尻子玉), a mythical ball said to contain the soul, which is located inside the anus. In the case of Reimu, she has a magical seal placed on her shirikodama peculiar to the Hakurei family that protects it from kappa.

With that context in mind your assessment of this image is as good as mine.
