Nothing's confirmed. People hint but never definitively state one way or the other because mystery makes her more interesting, there'd be no speculation or debate if there was a clear cut answer. And whatever early designs they have don't necessarily make it into the final cut. How many characters do you know are a 100% match to early concepts in terms of design, name, backstory, etc.? Things change, get retconned or simply get ignored in terms of canon all the time anyway.
Regardless, transgender people identify as the sex they see themselves as but I guess that counts for nothing. People will always indentify Poison as a he and demand a crotch bulge despite there being plenty artwork of futa of ladies or genderswaps of men. I'm just sick of that shit being matter of factly repeated by an army of parrots. Just fap and move on or something for fuck's sake.
