I think that's meant to be Corrupted Nero; a Nero that is taken over by the Velber's Anti-Cell and now serves the Umbral Star. See the markings on her left side? Or how her shield is now the Velber's diamond shape?
Now, a Nero Alter... I'm not sure, I don't know if her in the Rider class would be an Alter or not. On the one hand, it's her most powerful class, the one most fitting for her. On the other hand, Nero refuses to be in it since it has the greatest connection to the Beast Class as she summons Beast VI, the Beast of Revelation, as her mount.
For in the Nasuverse, Nero and the Whore of Babylon (or Mother Harlot) are the same individual, or have been made the same individual through association and misrepresentation.
