Ok, shot in the dark level 2. I do not possess any knowledge of Japanese so I'm guessing based solely on the artist's previous work and what I can discern from the picture.
1 - streaming video or recording to be released at sometime in the near future, probably if the weight in the pneumatic tube touches the lower button.
2 - when the weight touches the top button it will lower her hand restraints to get the key in the bell. Probably also her leg tethers will loosen.

Anon2, as far as I'm aware, the artist generally shies away from impossible predicaments. The girls might sometimes be forced to pick between shitty option A or shitty option B, but the task they're giving is almost never outright impossible, just very, very hard. The hole in the tube is probably just to make sure she can't rest for too long, since that would make it easier for her not to cum.

The artist's commentary for this image also explains that though the suction that sucks out any fluids from the onahole is not very strong, it's still strong enough to slowly suck out the air from the onahole, as well. So to maximize the amount of air she's pumping into the device, she has to thrust not only deeply, but quickly as well.
