She's not the one doing it. It'd be impossible for her to screw the screws without her thumbs. It looks like calligraphy class, perhaps they're permanently removing her ability to do so? A calligrapher maiden, maybe? It looks like ancient China or something, so maybe the Emperor wants her as his concubine but she refused. So, he removes her ability to write.
