Way back in medieval days, deodorant didnt exist as we know it. They relied on rare perfumes, that only he rich could afford, whereas peasants used things like herbs, flowers and fragrant fruits to smell better. Even so, it wasnt always available, as things like that were used for trade and food. Bathing wasnt a luxury either. Water was used for drinking and cleaning instead of washing. Clean water was valued for its purity, and most people wouldnt waste it on bathing. Obviously in those days, shaving was something reserved for men, so women just dealt with hair in all those places where we find it to be taboo these days.
She knows she smells, but there isnt anything she can do about it. Back then men loved their women, and probably appreciated every aspect of them, including their smell. Appreciating the smell of another person is a unique form of bonding. Though it might be offensive in some situations, its just a basic human function. And aside from pheromones, that putrid smell humans produce, can be a very erotic sensation. Almost a treat to some. I know for myself, if i could walk next to her, i would hold her hand and breathe deep, knowing that id stand by her side and fight with her through thick and thin. Regardless of her scent.
