Based on how enormous Kagefusa's breasts are in this OVA, each of her girls is probably storing a few gallons of milk. It makes sense why she'd react to the tight squeeze the way she did. Her breasts must be so engorged and painfully full. Thankfully Kagefusa gets a sweet milk release soon after this.
