Actually his first appearance was in RE:2 in a mini game, called "The Fourth Surivor" in the third one there was just and epilogue of him, in with the chopper pilot is saying something like "Once again only you survived, Mr. Death, it's always you to survive, Mr. Death."

Actually he was the the leader of the Umbrella security devision first squad. Ol birkin wasn't too happy about that mission and that's why only mr death survived.
