It takes place years after Metal Gear Solid. Metal Gear derivative mechs proliferate around the world thanks to Revolver Ocelot selling the plans to Metal Gear Rex. Solid Snake infiltrates an oil tanker to investigate a new Metal Gear that has been produced there and expose it to the world. However, the mission goes awry and the tanker is destroyed.

Two years later, rookie agent Raiden, the actual protagonist of the game, is sent to defuse a hostage situation at Big Shell, which was built to contain the oil spill caused by the tanker's destruction. The situation grows much more complicated and Raiden gradually learns that he is a pawn in a much larger scheme.

The game was made on a budget of 10 million dollars. The game went through quite a few plot revisions during development. The game's conflict was originally centered around nuclear weapon inspections in the Middle East, but the developers changed their mind due to then recent political troubles in the region. The last act of the game, which involved a lot of destruction in New York City, was changed significantly thanks to 9/11.

This game is notably one of the most controversial main Metal Gear games. While the game received strong critical praise upon release, the game's focus on Raiden was shocking to many fans as the pre-release material led many people to believe that Solid Snake would be the main character for the whole game. This, and various other aspects of the game, was met with considerable backlash and Konami would acknowledge the backlash against Raiden in Metal Gear Solid 3 with the look alike character of Raikov who the player gets to mistreat. Raiden would also return in Metal Gear Solid 4 and Metal Gear Rising Revengeance having undergone a significant change. At the same time, the game had ideas and plot points that would recur in subsequent Metal Gear games. It was the first game in the series where a no killing run was possible. The game also introduced a new group of antagonists who turned out to have a profound effect on the main Metal Gear storyline. Furthermore, some critics argue that this game was the first postmodern video game and was simply ahead of its time.

This game is famous for having an incredibly convoluted storyline, even by Metal Gear standards. The game's convoluted and enigmatic outcome, which Kojima originally intended to be left up to player interpretation, also inspired the creation of Metal Gear Solid 4 as many fans demanded answers to the mysteries this game's story created. Film composer Harry Gregson Williams worked on the soundtrack for this game and he would also work on the soundtracks for Metal Gear Solid 3, 4, and 5.
