It is the first canon sequel to Metal Gear and was released on the MSX in 1990. Hideo Kojima was not originally planning to make a sequel to the game, but he met a staff member who worked on the non-canon sequel Snake's Revenge on a train, and the staff member suggested he should make one. By the time Kojima got off the train, the basic concept of the game was born.

The world is suffering from a severe oil crisis and a shortage of petrol deposits. There does not appear to be any viable alternate energy source, but Dr. Kio Marv creates a new algae called OILIX that can solve this problem. Right before he is about to demonstrate this creation that can save the world he is kidnapped by troops and taken to the country Zanzibarland. Its leaders now has the world at its mercy as they have possession of OILIX and can determine how much they want to give to the rest of the world. They also have possession of nuclear weapons in spite of international calls for nuclear disarmament.

To solve this international crisis, Colonel Roy Campbell calls Solid Snake back out of retirement to infiltrate Zanzibarland and rescue Dr. Marv.

Despite being created in the 8-bit era, the game is stylistically closer to its more widely known sequels than the original in that it features a more complicated narrative. Metal Gear Solid and Metal Gear Solid 2's plots also share similar plot points with this game. Solid Snake's characterization is more fleshed out in contrast to his original near-silent portrayal. Snake in the original release did not look like his iconic appearance. The game was eventually rereleased as part of Metal Gear Solid 3 Subsistence which was the first time the game was officially localized for English speaking audiences. The game is predominantly the same but the character designs have been redrawn by Shinkawa Yoji to be more consistent with the Solid series. Some character names have also been altered.

The game expands on the stealth gameplay of the original, giving Snake the ability to crawl around and hide underneath objects and improving the guards' patrolling abilities. The guards now have a field of vision that is 45 degrees and can hear and respond to noises. These mechanics would be brought over to the first 3d entry in the series Metal Gear Solid.
