The people who drew this..clearly knew very little about horse anatomy, or just didn't care about where the dick actually went due to being too desperate to do the "it's sticking out under her sweater" thing on a horse girl. which just doesn't work.

I can't find it anymore, but I remember seeing some old depiction of centaurs where they had a human dick right before the horse part. Maybe that was the inspiration?

There is a manga (one of those about monsters girls and stuff) that a centaur girl talked about having "fake" genitalia infront (she was no futanari so she had a "fake" pussy) and was used as practice and the real reproductory stuff was in the back, in the horse part.
