Realbooru was conceptualized way too late. Perhaps if it had been made between 2005-2010 like Danbooru/Gelbooru/Rule 34 Paheal/Rule 34 xxx it'd be a much different story, but as is it'll never be big solely because it arrived too late onto the scene. There's also the issue that Booru/Rule 34 communities thrive specifically because they are relatively more niche in comparison to real porn so they carve out their own space easily, whereas real porn is as plentiful and mainstream as it comes with a million different ways and websites to consume it so it's less likely a community like that can form.
