1. Where the rating:SAFE ? The "rating:General" is not "safe", it is simple "unset"...
2. What the difference between the "rating:Questionable" and the "rating:Sensitive" ? I can't see the difference when I searchin images...

The "General" rating completely supplants the "Safe" rating; they have the same function.

This is the big mistake to make the "rating:General" as a default rating. Now it contains all types of ratings, including the explicit. A more intelligent solution is making a rating unset by default.

There is no default rating. Users must deliberately choose a rating on the upload page before it will allow them to upload.
When we did have a default rating, it was questionable, not safe or general.
