Size-wise I'd say melta. The lack of dedicated powersource attatched to the backback also points towards that.
Twin barrels usually points towards multi-melta, but there are a number of cases in which the additional barrel signifies the weapon's status as master-crafted (Karamasov's tri-barreled multi-melta comes to mind, as does Captain Invictus' plasma blaster and, of course, the ubiquitous combi-bolter).
