It's not about the method of restraint—or even the sex, really. It's about the horror. There is nothing more primarily terrifying than an injury that will never recover, and the knowledge that you're the one that caused such suffering is…sublime. For some people, this is better than any sex with her could ever be.

Sadism to this extent is the love for the context. The horror. Those that can actually bare to inflict such a thing are sick in the sense that they have a complete lack empathy. Although I have my own fantasies, I know they are enjoyed only in fantasy, where no one is really harmed.
I hope that MetalTxus really means the IDEA and nothing beyond the idea. Realistically, I can't fathom that level of inhumanity. The thought that it's possible for people to be capable of such a thing really deeply scares the fuck out of me.

Sadism to this extent is the love for the context. The horror. Those that can actually bare to inflict such a thing are sick in the sense that they have a complete lack empathy. Although I have my own fantasies, I know they are enjoyed only in fantasy, where no one is really harmed.
I hope that MetalTxus really means the IDEA and nothing beyond the idea. Realistically, I can't fathom that level of inhumanity. The thought that it's possible for people to be capable of such a thing really deeply scares the fuck out of me.

Nullname, it is not only people who have a complete lack of empathy who would be willing to do this outside of a fantasy. What matters is the way they empathize with people, and how it compares to the emotion felt when inflicting such horrors. For some people, it's empathy itself which makes it more enjoyable. Sure, blood and tears alone can be pretty nice, but there's nothing that can describe the feeling of attachment for someone you hurt, and that is not possible without empathy. The feeling anon2 beautifully described would be dulled for someone who feels nothing when inflicting pain and only enjoys the visceral sensations.
Also note that, just because there are people who would enjoy this even outside of a fantasy, they are not necessarily unable to control themselves. Sometimes remaining a free man is more important than transient pleasure.
And anon2, she's not underwater. Those clouds are smears on the glass. Don't be stupid.
