Decensored typically refers to an image that was censored but has since been edited to remove censorship. Uncensored refers to an image which was was never censored to begin with.

Because she is drawn with the body type of a child in this image and it is rated Explicit. Her actual age means nothing for the tag. This has to be explained all the time, it's not a difficult concept.
