The difference between onlookers and voyeurism is that voyeurs watch others stealthily, without the knowledge of the people who are being watched. And voyeurs often look from a certain distance or somehow hidden (through a keyhole, behind a door, behind a wall, behind a tree, etc). With onlookers, however, the people being watched aware of the onlookers' presence. Also, onlookers tend to be close to the action and in plain sight.
