The game is notable for having a sneaky advertising campaign. The original trailer presented the game as if it were a stand-alone survival horror game called The Phantom Pain made by Swedish studio named Moby Dick Studios headed by a man named Joakim Mogren. However, fans picked up hints in the original trailer that suggested that it was actually a Metal Gear game in disguise and on March 27, 2013, Kojima revealed at the Game Developer's Conference that Phantom Pain was in fact Metal Gear Solid 5 and that Moby Dick Studios and Joakim Mogren were completely made up. Kojima explained that he initially presented The Phantom Pain as a stand-alone game in order to get an objective evaluation of the new Fox Engine that the game uses.

The game is directed by Hideo_Kojima. Artwork is done by Shinkawa_Yoji and music is composed by Harry Gregson Williams, Honda Akihiro, and Ludwig Forssell. The story is written by Kojima, Murata Shuyo, and Inamura Hidenari. The game is split into two parts and is essentially two games that combine to form a larger story. They are released for the Playstation 3 and 4, and the XBOX 360 and One. The first shorter part is called Ground Zeroes and serves as the prologue to the main story called The Phantom Pain. Ground Zeroes was first released on March 18, 2014. Phantom Pain was released on September 1, 2015. The game cost over 80 million dollars to make. Like most of its predecessors, the game received strong critical praise.

The game is set in 1975, shortly after Metal Gear Solid Peacewalker's conclusion. As the game is a direct sequel, those who haven't played Peacewalker should be wary of spoilers. Furthermore, the game also has quite a few plot twists and revelations that cast a different light on the franchise as a whole, so players who wish to avoid spoilers should be wary of looking up information on this game. Big Boss learns that two of his younger comrades are being held in an American base on Cuban soil called Camp Omega and he infiltrates the base to rescue them. Unfortunately, while Big Boss is busy infiltrating Camp Omega, MSF's Mother Base is attacked and destroyed by the members of a new shadowy organization called XOF whose leader Skull Face proves to be much more ruthless and malevolent than any enemy that Big Boss has previously faced. Not only is MSF destroyed, but Big Boss's rescue attempt goes horribly wrong and Big Boss falls into a coma and is separated from his companions.

9 years later, Big Boss reawakens, but things are no longer the same. With the loss of Mother Base, the MSF is a shadow of its former self. Big Boss has suffered numerous injuries and due to spending a great deal of time in a coma needs time to reacquaint himself. After escaping from a hospital with the help of a mysterious man named Ishmael, Big Boss is brimming with anger and a thirst for vengeance against everyone responsible for MSF's destruction. Big Boss assumes a new name Punished Snake, forms a new mercenary group called Diamond Dogs, and heads to Afghanistan to pursue his vengeance. During his pursuit of revenge, he uncovers a plot by a mysterious organization called Cipher, which aims to create a superweapon more powerful than the Metal Gears. Big Boss sets out to foil Cipher's plot and get his revenge, but he finds himself losing his humanity in the process of doing so. . .

The game is a stealth game with elements of a sandbox game. The game is designed to encourage more than one way to stealthily infiltrate enemy territory and the environments are considerably larger.The game retains the base-building system and mission progression structure from its predecessors Portable Ops and Peacewalker. Big Boss can do things like ride vehicles and pretend to be one of the enemy vehicles driving around bases as well as ride horses. After they are unlocked, the player can choose which characters to bring along on missions for assistance. The game also has day and night cycles. This game also handles cutscenes differently from past Metal Gear games, cutscenes this time are animated to resemble elaborate and lengthy one-take shots in live action films.

The game's story is notably the darkest of all the Metal Gear games with characters committing crueler acts of war-time brutality as well as showing Big Boss's ultimate descent from hero to villain with many allusions to the novel Moby Dick, which was also a tale of man being consumed by vengeance. Just as Metal Gear Solid 4 acted as the finale to Solid Snake's story, this game serves as the finale for Big Boss's.

Like past 3D Metal Gear games, the game uses motion capture to bring the characters to life. However, this time Kojima decided to use English speaking actors to do the motion capture work with the Japanese seiyuu dubbing over the English dialogue delivered by the actors. While Otsuka_Akio reprises his role as Big Boss in the Japanese voice track, Kojima made the controversial decision to have English voice actor David Hayter, who is most well known for playing Solid Snake and Big Boss (except in MGS4) in the English dubs of the Metal Gear franchise, replaced by film/TV actor Kiefer Sutherland who also did the motion capture work for Big Boss. Kojima wanted an actor who could emote "through subtle facial movements and tone of voice rather than words" as well as an actor who "could convey the facial and vocal qualities of a man in his 40s." Sutherland was recommended to Kojima by film producer Avi Arad.

Before the game's release, Kojima had a serious falling out with Konami, which led to Kojima leaving the company and Konami removing direct references to Kojima and Kojima Productions from promo material for this game. Kojima Productions disbanded as well. As a result, this game acts as a finale to Big Boss's story arc and Kojima's tenure as the director of the Metal Gear franchise. This also led to a large amount of material planned for the game to remain unused.
