Well, when he says he lost his "v-card" to Erika, he does say he noticed something weird. However, because of what was happening to Vincent at the time, he never got to explain what it was.
Maybe they went anal with the lights off and he was a bit drunk and nervous to rationalize what was happening.
As many transgender don't take the final surgery step (with fear of loosing sensation and other complications, as well as steep price), maybe that's the case for Erika.
There's no evidence in the game, so one can only speculate.
