Actually, FMA isn't the only or first anime to have characters with mechanical limbs because Vash the Stampede from Trigun had a mechanical left arm with a built-in gun and Trigun was finished 4 years before FMA even started to be published.
I don't know if Trigun was first, though i doubt it, but either way FMA isn't either the first or the only anime to have a character with a mechanical limb.

Cobra and Berserk are some other anime/manga that feature protags with artificial limbs both came out around the 70s, 80s
