Anon 1，that's basically an artificial lifeform called a "Sexbeast" she glomped onto her body to make her fight better. That critter's got feelers that feeds on her...um, "essense", at where where it contacts her...um, points.
And yes, they have male and female Sexbeasts, so this is why you get a tasty lil' futanari action in the first chapter of the H-manga she stars in.
