It's a side effect of Gelbooru's tagging policy. This girl is androgynous. The term has always meant the character had both masculine and femine traits or otherwise none so you would have trouble discerning their sex. Even knowing their sex beforehand doesn't mean they aren't androgynous. I'm pretty sure the reverse trap tag doesn't apply though since you can tell she isn't a boy, even from the thumbnail.
