With a Miko, you a slight conservative clothed woman that leaves a lot to imagination similar to a Catholic nun with no habit.

At the same time, there isn't as much stopping a Miko from having a sexual relationship.

The outfit is cool, and it can be fetish fuel too. And I have no problems with that.
