If this is from the anime I think it is from, then this is not futanari but a trap. Or a futanari genderswap at least

In a certain chapter in the offical manga, Ichiko actually was able to possess Mizuho and turn her fully female. So this could be a trap or a futanari version of that.

This is a modified idea based on a scene from the anime.

During one episode, Mizuho has to change for gym, Mariya catches him looking at others, so she goes to gropes his falsies. xD But he was still in the school uniform at the time.

Interesting fact, except for a few comments by characters, you never see Mizuho as a guy or even in boyish clothes. You even see Mizuho in bikinis and swimsuits and cant tell he is male.
