The top hat and the line in the middle of the gothic lolita girl's face suggest that it is the personification of Napstablook and not Mettaton. Unless the artist says otherwise, I'm pretty sure it's Napstablook.

Wish I could edit what I said as anonymous #1868934. I checked the source link. The artist wrote in the description that it is Napstablook drawn with a gender neutral look. So it is Napstablook with Mettaton accessories and not Mettaton himself. Case closed.

Correct. Plus, the simplest way to figure out is by how this character dress. All white clothes, hair, eyes, top hat, and headphone. They also are a huge fan of Mettaton, so of course they have Mettaton's accessories. There are also snail's shells in the foreground.
