Her childhood nickname is Anaru, which is pronounced more or less the same way a Japanese person would pronounce "anal". It's a subtle-ish joke in the anime, where she's old enough now to understand how other people might hear her nickname and doesn't want people to call her that anymore. She's not shown being sexually active, though - the opposite, actually - so I wouldn't say that she's any sort of "anal whore" in the anime, much less that she qualifies as "No.1".

She might be the top anal character in hentai, though. I wouldn't be surprised if she's always shown having anal sex in doujinshi.
