After Gwyndolin was born linked to the Moon, which was linked storywise to femininity, "good" daddy Gwyn made Gwyndolin live as a woman via magically binding a Ring of Reversal on him which forced him to act like a woman as well, and probably did other changes over the hundreds or thousands of years he wore it. Thus, Gwyndolin was raised as a woman, including magical enforcement of him being a woman.
Eventually he was able to take the ring off (we find it in the Darkmoon Temple in Anor Londo in DS3), but still was very feminine, including small breasts, slender but feminine hips, a pretty face, and an androgynous voice.
