A psychiatrist won't help you against cancer pains or other similar conditions. In poor countries people are forced to buy drugs from drug dealers, b/c medics do not treat terminal patients and there is no hospice help.
It also won't help if you don't have depression/mental illness per se, just don't see a point to go on.

To be honest i don't like the shotgun idea, it seems just a messy and nasty way, i prefer the single shot, it makes it look beautiful, there's some certainty in the single bullet.
