It's a bit of a subplot in the manga that Belarus is batshit insane, yes. But since she only wants to become one with Russia and doesn't even look at other men, I think it's safe to say she's not a whore. >.>

The point of the picture relies on the characterization and relations of the representatives, not the countries they supposedly depict. It could even be possible the picture isn't meant to be humorous at all, but simply to parody a scene from Higurashi no Naku Koro Ni, whatever that is.
