Do those artist look on the final image and after thoroughly checking, seriously say "Yep, good to go!"?
I mean, some minor issues, yeah, sure, happens, but something blatantly obvious like that?!

yeah. that is pretty bad. something about it was bothering me immediately, but it wasn't till i read your comment that a started looking and found what was upsetting me

I imagine they started with the foot, then at some time during the creation process they realized they wanted to do a different pose, but by the time they were finished they forgot to change the foot.
