Angewomon, digivolves into both Magnadramon (in the movie-not sure which right now- Both Angewomon and MagnaAngemon digivolve so that the Golden Digieggs will appear). That's a Adventures 02 movie.

In Digimon Frontier Angewomon digivolves into Ophanimon.

OK, I just looked into Gatomon's forms because my memories of Digimon are kinda fucked up because of thepassage of time (need... sand... now...). The chart is mostly correct, but Aquilamon is Hawkmon's Digivolution, not Gatomon's, and Ophanimon has not been put here, as stated above by others.
