A truly fearsome creature (or at least a more efficient one) if she were able to take control even if the human awakens.
I could see advantages to it.
Also, regarding the 'front vag' question, that would explain why she covers up so much. But that could well be an odd piece of anatomy.

I would assume depending on which end you use, that one will/may result in Pregnancy. Then again, it happens all in dreams so that complicates matters. Again though, it's just trivial cause of the dream/magic Scenario.
Also, I would like to add that the Scythe is used to rip/create a whole/portal into a desired persons dreams, not used to mame or injure.
