"Realistic" assuming people can be hanged by a cloth under their chin.
Why do so many artists draw asphyxiation like the human neck is inflexible and the human body is rigidly locked in an upright position by fucking magic? I don't think you could hold yourself in this position ON PURPOSE, as a feat of strength. Maybe lunatic free-climbers can do a pull-up using only their chin - but guro artists have some secret agreement to pretend an unconscious person wouldn't immediately fall out of this.
