well the character suffers from a sort of narcolepsy, but while unconscious/comatose she doesn't age. so she looks 12 but is actually 19. in this particular depiction i'd assume people are harmlessly changing her underwear for her as she cannot possibly do it herself due to the sudden onset of her condition.
