Starting a quest in a town pretty much guarantees mission clarity failure - the players will just fuck around in the town until one of them picks a fight with a civilian and the GM sends in the cops, who take the PCs to the Lord, who tells them if they don't want to rot in jail they must complete a mission because GODDAMN IT I WORKED ON THIS QUEST FOR A MONTH

For every edition after 1st that actually matters (meaning 4th may as well not exist) Charisma DOES equal appearance. But not just appearance. Charisma is supposed to be a combination of physical appearance and presentation, as well as force of personality.
People skills are determined by skills. Hence people "skills". Dealing with people isn't instinctive or innate, it's something that people start learning from an early age yet it is no less a skill for how ingrained it is. These skills in D&D are usually Diplomacy, Bluff, Intimidate, Gather Information, and Sense Motive. Those are your people SKILLS.
