Tolone is a cyborg with cybernetic implants in her brain (they make a massive Star Trek joke about it right after she's first introduced). She actually can remove one of her hands and use her arm like an arm cannon, too. Anyway, that's why she has screws in her face. You can see them in the in-game portraits too.
All of Ramsus' Elements are oddballs in some way. Seraphita is a demihuman with bunny ears and a tail, Dominia is the last survivor of a civilization Id obliterated and Kelvena has Ether powers so strong she has to keep her eyes closed when she's not fighting to keep control of it.
