Personally I'd say she banged her Head pretty bad when EVA-03 turned into Bardiel and EVA-01, posessed by the Dummy System, went guro_tag on it with her still trapped inside.

In the Preview of 3.0 you can see her wearing the eye patch in a pose like this.
