the game kind of does go this far. Maybe not physically but it doesn't need to. Upon entry you're kindly asked to leave, she made a sanctuary for the diseased, she you won't find anything here worth taking. She doesn't want to fight you and then you go down there and take matters into your own hands. You can talk to her after killing garl or do what this guy in the picture is doing.
