Because in anime, feminine-looking = necesarily female, amirite?
Naruto (as most other characters) has been drawn as a bishounen before, and that doesn't make it a genderswap. This is Naruto when he imagined how would he look with his mother's hair. The effimenate look, is just the artist's prefference.
