I only figured out that Envy was supposed to be a man when Ed referred to Envy as a "he" (I'm pretty sure Ed was in the hospital at the time).
When I first saw him, I honestly couldn't decide which he was.

To be precise, Envy should technically be a male in the 2003 anime, where he is actually (the product of a failed human transmutation of) the son of Hohenheim and Dante.
In the manga and Brotherhood, however, Envy is genderless.
