Not shown, just referenced. She calls herself a freak and when you're trying tot kill that Shade that killed her grandmother, the villagers keep saying "you know what you are"(if I remember right). It's not an overly important part of the story.

After beating the game the first time, you get to know a LOT about Kaine's background, one of which is said that she was born transgender. so yes, Kaine being a hermaphrodite is cannon.

They're not human, they were made by the real humansm who are now shades, you could call them Replicants, that's what they're called in the game in reference to Blade Runner, so when they created Kaine from the original woman there was a bug in the program and she was born with a penis, and not because a male shade possesed her like many people think.

They didn't wanted to tell all this in the game so you don't lose focus of the main plot, but everything is explained in Grimoire Nier.

Not "born trans" she was born Intersex, some part of her is male at birth. in some costumes she has a very noticeable bulge, so it's easy to guess what that part is.
