Agreeing with #363385, as that very much looks like the Meedle hospital and Cloud's less than vibrant eyes there, especially compared to Tifa's, imply his Mako poisoned state. Also the fact that he's completely limp except for his boner.

That said, I have a feeling that the doctors are watching from around the corner somewhere.

Also, she's crying because she has to do all the work, and was expecting their sexy time to be him pounding into her hard and fast, and her position kind of prevents that.
