I dunno if this can applies to manga but here:

the manga author seem to have been inconsistent on drawing this character, which led to the gender confusion (not to mention it uses " Boku - ボク " a boyish way of (describing yourself/sayiing "I")
though there are a lot of girl-characters in other manga series that does this they are genrally called "bokuko/bokukko"

another culprit is Killua's little-brother(so I heard) Kallut Zoldyck (who dresses in drag and also uses boku)
Nelfelitou's "pear body-shape" have been shown in early chapters, specially in the latest chapters.
this might be the reason on the boost on female-Nelfelpitou fan arts,though some still remains neutral about the gender until further confirmation (like me, coz the laws of manga can go beyond science).
some even go so far to say that pitou uses "dr. blight" to change gender all the time.
