If you look closely you can see two distinct heads in the creatures "ball sack" (in quotes because it doesn't see to actually have testicles). One of them is smooshed towards the bottom of the picture and the other is the one you can see clearly.

No, they're not both in the ball sack.
It's not quite logical, but the fair haired guy who went the oral way is the one who ended up in the beast's scrotum. There is no one else there. You can see for yourself if you visit this site which has a very clear enlargement - 1600-1004 pixels! - of the scrotum and its contents.

I wish there was more information on this comic. What are these men, dragon slayers? The story seems to miss a few frames, I felt like I was thrown smack in the middle of a narration.
And why is this story not included in one of Mentaiko's album publications? Why doesn't it have a title? More puzzling: why is there only one website featuring that specific scrotum detail, where did the moderator get it?? It's actually the very same drawing as in frame 7, except that the two reflecting light spots are not there because the ball-sack skin is absent, the scrotum is open, so to speak.
I think that this (together with the sequence of the man-tiger who gets swallowed & raped by a carnivorous plant) belongs to Mentaiko's best work. Why didn't he continue in this genre? I certainly want a Prequel plus a Sequel of the delightful Dragon story!
And what is this Japanese obsession with censorship on frontal nudity? In such a senseless form! The white scratches hardly obliterate the genital areas, but they do ruin the beautiful artwork!

I once found the final frame - there are 8 frames in total - at the address you gave. I downloaded it, and for years was the only one on the planet who had the complete comic! (except for the artist, of course). Seven frames are uploaded at various sites, but never that final one with the closeup of the beast's scrotum. And it's a stunner!
I also love the three frames in which the dark haired crusader slides head down into the slimy hot beasts's uterus until only his bare feet are left visible, I had a little naughty fantasy about his trip...
Guys, this is Mentaiko Itto's best comic. And while perhaps an overture may be missing (I suspect), the whole work is beautifully drawn and colored and the floating clothes and weapons of the dragon slayers (?) create a comical but also elegant effect.
Why o why did Mentaiko never publish it in one of his many albums??
And to avoid censorship he should have it published in France, or even America. The ugly white scratches over genitals;' Itto had to do that, the Japanese law is very strict. But it ruins his work!
I wish I could communicate with the artist. Did anyone ever do that? It's a rare, untitled work, quite different from what he usually does (the Priapus stories). And although there is no sex in it, it's damned erotic!
