Using trusty ol' google translate and looking at the other images with the same tag suggested it was a scale of gender or something, left being fully male and right being fully female.

It's simpler on the other images as none of them are as androgynous as Lilith.

Correction - the one on the left reads as 'flat-chested' (as in, A-cup, not typical male flat-chestedness) and the right reads as... well, it's obvious. 'Well-endowed'. Not genderswap.

Lilith is suposed to be the "good" part of morrigan who manifests as a naive young girl. Her sex appeal is thought to be different than morrigan, she´s a loli with a very slim yet curvy body, in some way her flat chest is ironically rounding the idea.
